Valley Health Center for Senior Wellness in Huntington, WV is seeking a Nurse Practitioner or Physician Assistant to provide comprehensive healthcare services with an emphasis on home visits for seniors. The role involves building trusted relationships with patients and families to deliver accessible, patient-centered care in a community-based setting. This full-time position offers a day shift schedule and a comprehensive benefits package.

Responsibilities

• Conduct home visits to assess, diagnose, and manage patients' health conditions
• Develop, implement, and evaluate care plans in collaboration with the care team
• Provide preventive care, chronic disease management, and patient education
• Collaborate with physicians, nurses, and other team members to ensure continuity of care
• Document visits and maintain compliance with regulatory standards

• Salary-based compensation (W-2 tax status)

Requirements

• Current licensure as a Nurse Practitioner or Physician Assistant in West Virginia
• Certification and prescriptive authority required

• Ability to work independently in the field while staying connected to the care team
