Lone Star Family Health Center is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to provide outpatient primary care. The role involves conducting health assessments, diagnosing and treating common conditions, prescribing medications, and supporting patient education within a collaborative team environment. Responsibilities

• Conduct thorough health assessments, develop personalized care plans, and monitor patient outcomes.
• Diagnose and treat common health conditions within scope of practice.
• Prescribe medications from the institutional formulary.
• Provide support to patients managing chronic conditions and guide families through health journeys.
• Connect patients with community resources and collaborate with healthcare team members.
• Educate patients and families on health management and preventive care.

Requirements

• Current licensure through the Texas Board of Nurse Examiners (FNP) or Texas State Medical Board (PA).
• Preferably two (2) years of outpatient clinical experience as an FNP or PA.
