The Advanced Practice Clinician (NP or PA) will provide in-home medical and social support to senior patients enrolled in the Care Anywhere program. The role focuses on conducting assessments, managing care plans, and coordinating with healthcare providers to prevent hospitalizations and manage chronic conditions, primarily serving the elderly population in a remote setting.

Responsibilities

• Conduct in-home assessments, including medication reviews and health screenings for patients.
• Provide patient education tailored to individual care needs.
• Identify diagnoses relevant to care management and active medical intervention.
• Collaborate with Regional Medical Officers and care teams to develop appropriate care plans.
• Coordinate care with primary care physicians, specialists, and ancillary providers.

Compensation

• $130,332.00 - $195,498.00 annual salary

Requirements

• Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
