An autonomous Advanced Practice Provider role specializing in in-home wound care is available in Gary, Indiana. The position involves conducting patient assessments, diagnosing conditions, and developing care plans, with a focus on work-life balance and community impact. The role offers flexible scheduling, travel between patient homes, and comprehensive support and training.

Responsibilities

• Conduct comprehensive patient assessments in home settings
• Diagnose conditions and develop individualized care plans
• Maintain timely and accurate documentation within 24 hours of patient visits on EHR
• Travel between patient homes within a designated territory with mileage reimbursement
• Utilize provided equipment and supplies to deliver wound care treatments
• Collaborate with the team to ensure holistic patient care
• Stay current with training and continuing education requirements
• Ensure compliance with clinical protocols and documentation standards

Compensation

• Highly competitive base salary with uncapped RVU bonus structure, with proven earnings of $175,000+
• Productivity-based compensation model (wRVU)
• Additional income through RVU bonuses and CME allowance of $2,000

Shift & Schedule

• Monday through Friday schedule
• Approximately 7:30 am to 3:30 pm or four 10-hour days

Requirements

• Active and unrestricted Physician Assistant (PA) certification
• Graduate of an accredited physician assistant program
• At least one year of experience as an Advanced Practice Provider (APP)
• Clinical background in adult, family, geriatric, surgical, or wound care preferred
• Willingness to obtain multi-state licensure (covered)
• Experience with comprehensive patient assessments and documentation
• Ability to work independently and manage a flexible schedule
• Excellent communication and organizational skills
• Comfort with travel and driving between patient homes
• Additional wound care training provided if needed
