The Nurse Practitioner (NP) provides comprehensive hospice care under physician supervision, including patient assessments, symptom management, and care planning. The role involves collaborating with interdisciplinary teams to deliver patient-centered end-of-life care in a home setting, ensuring compliance with regulatory standards and maintaining high-quality clinical practice.

Responsibilities

  • Provides medical care and symptom management for terminally ill patients in compliance with state regulations and physician supervision.
  • Conducts comprehensive history and physical examinations, orders and interprets diagnostic tests, and documents findings.
  • Develops individualized care plans, prescribes medications and treatments, and coordinates referrals.
  • Completes face-to-face assessments for hospice recertification and provides clinical eligibility information to the medical director.
  • Collaborates with the interdisciplinary team to develop, review, and update care plans.
  • Consults with physicians and other healthcare professionals regarding patient care and eligibility.
  • Educates and supports patients, families, and caregivers on medical, psychological, and spiritual issues related to end-of-life care.
  • Ensures compliance with hospice regulations and participates in quality improvement initiatives.

Requirements

  • Current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in applicable state(s).

  • Valid state driver’s license, access to a reliable automobile, and appropriate auto insurance.

  • Knowledge of hospice philosophy, end-of-life care, and patient-centered treatment.
  • Ability to work independently with minimal supervision.
  • Fluency in English (reading, speaking, writing).
