The Hospice/PC Nurse Practitioner (NP) is a master's prepared, licensed advanced practice registered nurse in California, responsible for providing hospice and palliative care through assessment, diagnosis, treatment, and care management. The NP collaborates with physicians and the interdisciplinary team to deliver holistic, family-centered care to seriously or terminally ill patients, serving as a team leader and clinical mentor.

Responsibilities

  • Provide medical care to seriously or terminally ill patients in accordance with the patient's plan of care and collaboration with physicians and medical directors, including performing history and physical exams, diagnosing and treating medical problems, ordering and interpreting diagnostic tests, and educating patients and families.
  • Deliver skilled nursing services aligned with the patient's plan of care, including assessments, symptom management, and health education.
  • Assess physical, psychosocial, environmental, safety, and developmental needs of patients and families, and individualize care plans accordingly.
  • Manage pain and other symptoms, developing and implementing individualized pain management strategies.
  • Support and educate patients, families, caregivers, and staff to meet patient needs and ensure quality care.
  • Document assessments, care provided, interventions, and patient outcomes in compliance with agency policies and regulatory standards.
  • Supervise nursing staff such as RNs, CHHAs, and LVNs as required by law and policy.
  • Participate in interdisciplinary team meetings, provider on-call rotations, and administrative activities, while maintaining patient confidentiality and professional conduct.

Compensation

  • Pay is $150 per face-to-face visit plus $11 for mileage, totaling approximately $161 per visit.
  • Compensation is provided via W-2 employment.

Requirements

  • California-licensed Nurse Practitioner with current certification and licensure.

  • Current CPR, TB clearance, and health screening.
  • Knowledge of CMS standards and regulatory requirements.
  • Excellent verbal and written communication skills.
  • Ability to perform physical assessments and manage patient care in various settings.
  • Valid automobile insurance and a safe driving record for travel between patient locations.
  • Proficiency in English, both spoken and written.
  • Ability to work independently and as part of an interdisciplinary team.
  • Must have visual and hearing acuity necessary for patient care; physically capable of walking, standing, and assisting with patient transfers.
