Porter is seeking Nurse Practitioners to provide in-home health assessments within Lucas County, Ohio, with secondary coverage in Wood County. The role involves conducting comprehensive evaluations, including medical history, physical exams, and point-of-care testing, to support care management and improve patient outcomes. Responsibilities

  • Conduct in-home health risk assessments for patients within the geographic area, including medical history review, physical examination, and vital signs measurement.

  • Identify diagnoses based on clinical findings to support care management efforts.
  • Reconcile medications and review systems as part of the assessment process.
  • Document findings and formulate a list of current and past medical conditions.
  • Collaborate with Porter Care Guides and medical team to develop and implement care plans.
  • Travel within designated areas, including secondary coverage in Wood County.
  • Maintain accurate and timely documentation of assessments and findings.

Compensation

  • Fee-for-Service pay structure based on completed patient assessments.
  • Hourly rate of USD 115.00, paid per assessment.
  • Paid mileage for travel over 20 miles per day.
  • Competitive wages with potential to increase earnings by seeing more patients.

Requirements

  • Valid, unencumbered Ohio Nurse Practitioner license.
  • Minimum of 3 years of patient care experience, preferably in primary care, adult, or geriatric populations.

  • Strong sense of empathy, compassion, and friendliness.
  • Excellent written and oral communication skills.
  • Ability to work independently and manage a flexible schedule.
  • Reliable transportation for travel within assigned geographic areas.
  • Ability to perform point-of-care testing and physical assessments.
