The Nurse Practitioner/Physician Assistant (per diem) will provide primary care services to Optum members in home, nursing home, or assisted living settings. The role involves managing medical and behavioral health conditions, conducting assessments, documenting in compliance with CMS regulations, and coordinating care transitions. The practitioner collaborates with facility staff, families, and physicians to develop and revise care plans, ensuring quality and cost-effective care delivery.

Responsibilities

  • Perform comprehensive assessments, document findings accurately, and ensure compliance with CMS and documentation standards.
  • Conduct initial, annual, and post-hospitalization medication reviews and reconciliations.
  • Facilitate agreement and implementation of the member’s care plan with facility staff, families, and providers.
  • Evaluate and revise care plans based on member needs, effectiveness, and cost considerations.
  • Assess the medical necessity of ancillary services and coordinate care transitions across different levels of care.
  • Communicate effectively with members, families, providers, and internal teams regarding care plans and program policies.

Compensation

  • Hourly pay ranging from $43.22 to $77.21 based on full-time employment.
  • Sign-on bonus of $3,500 for external candidates.
  • Additional income opportunities include equity compensation, CME allowance, referral incentives, and practice allowance.
  • Compensation is provided through W-2 employment.
