The Nurse Practitioner in Palliative Care provides comprehensive medical and social assessments, manages treatment plans, and offers health education to seniors in a home health and hospice setting across Jackson, Pike, Scioto, and Gallia counties. The role involves independent clinical decision-making, coordination with healthcare providers, and patient follow-up to ensure quality care and symptom management.

Responsibilities

  • Conduct physical exams and obtain comprehensive medical and social histories of patients, including those with acute and chronic conditions.
  • Order, perform, and interpret laboratory tests for screening and diagnosis.
  • Prescribe medications, recommend diets, and initiate treatments such as physical therapy.
  • Perform therapeutic and urgent care procedures as needed.
  • Refer patients to specialists or other healthcare providers when appropriate.
  • Provide health education, counseling, and instructions on health maintenance, illness prevention, and medication use; evaluate and follow up on patient compliance.
  • Manage patient care plans, monitor progress, determine need for referrals or hospital admissions, and assess when illnesses are resolved.
  • Serve as a preceptor for medical, nursing, NP, or PA students and collaborate with other healthcare providers regarding patient care and discharge planning.

  • Salary-based compensation (W-2 tax status).
