PACE North is seeking a Nurse Practitioner or Physician Assistant to provide primary care services to older adults within a community-based, multidisciplinary team. The role involves delivering holistic care across multiple settings, including clinics, homes, and skilled nursing facilities, with a focus on person-centered, flexible care for participants aged 55 and older.

Responsibilities

• Collaborate with a multidisciplinary team to address the emotional, physical, social, and medical needs of participants.
• Deliver care in various settings: on-site clinic, participant homes, and skilled nursing facilities.
• Participate in a rotating after-hours on-call schedule to support the on-call nurse.
• Build strong relationships with participants and their families through education and support.
• Practice in a flexible care model that adapts to participant needs rather than rigid systems.

Requirements

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current and valid Michigan license as a Nurse Practitioner or Physician Assistant.
• Strong clinical leadership skills.
• Excellent interpersonal and communication skills.
• Effective problem-solving abilities.
• A compassionate, collaborative, and person-centered approach to care.
• Experience in primary care or geriatric care (preferred).
• Ability to work within a flexible, participant-focused care model.
