The Nurse Practitioner (NP) provides comprehensive hospice care under physician supervision, including patient assessments, symptom management, and care planning. The role involves collaborating with interdisciplinary teams to deliver patient-centered end-of-life care in home and facility settings, ensuring compliance with regulatory standards and maintaining high-quality clinical practices.

Responsibilities

• Provides medical care and symptom management for terminally ill patients in compliance with state regulations and physician supervision.
• Conducts comprehensive history and physical examinations, orders and interprets diagnostic tests, and documents findings.
• Develops individualized care plans, prescribes medications and treatments, and coordinates referrals.
• Completes face-to-face assessments for hospice recertification and provides clinical eligibility information to the medical director.
• Ensures effective symptom control through collaboration with the interdisciplinary team.
• Participates in interdisciplinary meetings, maintains accurate medical documentation, and updates care plans.
• Educates and supports patients, families, and caregivers on medical, psychological, and spiritual issues related to end-of-life care.
• Ensures compliance with Medicare, Medicaid, and federal/state hospice regulations, and participates in quality improvement initiatives.

Requirements

• Current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in applicable state(s).
• Degree from an accredited nursing and Nurse Practitioner program.
• Minimum of two years of RN experience.
• Ability to pass background checks, consumer reports, and drug testing.
• Valid driver’s license, access to a reliable automobile, and acceptable driving record.
• Proficiency in electronic health records and documentation.
• Strong communication, observation, and problem-solving skills.
• Knowledge of hospice philosophy, end-of-life care, and patient-centered treatment.
• Compassionate, professional, and able to maintain a calm demeanor in patient interactions.
• Physical ability to lift up to 30 pounds regularly and up to 50 pounds occasionally.
