Hospice de la Luz / Jet Health is seeking a PRN Nurse Practitioner to provide face-to-face hospice care visits in the Rio Rancho, Belen, Los Lunas, Albuquerque, and surrounding areas. The role involves collaborating with the interdisciplinary team to oversee clinical care, educate patients and families, and ensure quality end-of-life care with flexible scheduling and autonomy.

Responsibilities

• Collaborate with the Interdisciplinary Group (IDG) to evaluate and develop palliative treatment plans for terminal patients.
• Consult with and assist in the development of patient and family care plans to ensure quality care.
• Educate patients and families about disease processes, self-care, and end-of-life issues; provide staff training as needed.
• Perform and document patient/family visits based on the plan of care and patient needs.
• Communicate with attending and consulting physicians regarding palliative care and patient progress.
• Provide education and clinical guidance to other IDG members on palliative care issues on a PRN basis.
• Participate actively in IDG meetings and communicate effectively regarding patient status and care outcomes.
• Inform physicians and team members of changes in patient needs and responses to interventions.

Compensation

• Per visit face-to-face rate for hospice care visits.
• Additional monthly stipend for on-call duties, IDG meetings, documentation, and related activities.
• Mileage reimbursement for travel between patient locations.
• Practice allowance included as part of compensation.
• Tax status: W-2 employment.

Shift & Schedule

• Variable schedule with flexible hours.

Requirements

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse and Advanced Practice Nurse.
• Valid Driver's License and reliable transportation with auto liability insurance.
• At least two years of experience as an APN, with preferred experience in hospice or home health care.
• Strong nursing skills aligned with accepted standards of practice.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life issues.
• Good interpersonal and communication skills.
• On-call availability 1-2 weeks per month, with flexibility to work variable schedules.
• Ability to work in patients' homes under various conditions, including exposure to blood, bodily fluids, and infectious diseases.
• Ability to travel locally and work flexible hours, including some exposure to weather conditions.
