This part-time, W-2 Nurse Practitioner or Physician Assistant role in Physical Medicine and Rehabilitation (PM&R) is based in Washington Court House, Ohio. The clinician will provide specialized rehabilitative services at a skilled nursing facility, collaborating with interdisciplinary teams to optimize patient outcomes. Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant at a skilled nursing facility.
• Represent CRC through clinical leadership and collaboration with the interdisciplinary care team.
• Assist in managing conditions common in SNFs, such as CVA, neurological disorders, and postoperative orthopedic issues.
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Track patient progress throughout therapy and assess barriers to therapy, including pain and neurological issues.
• Participate in multidisciplinary meetings and bridge communication between therapy and internal medicine.
• Engage in ongoing education, including monthly lectures, virtual meetings, and regional events.
• Contribute to a positive facility culture and support company growth initiatives.

Compensation

• Hourly rate starting at $75.00 per hour.

Requirements

• Nurse Practitioner, APRN, or PA license in Ohio.
• Ability to work in the U.S.
• Experience in post-acute or SNF settings preferred.
• Confidence to work autonomously.
• Strong interpersonal skills and empathy.

• Willingness to work weekdays, Monday through Friday, with flexible hours around 9 AM - 4 PM.
