AuthoraCare Collective is seeking a full-time Nurse Practitioner or Physician Assistant for Home Based Primary Care in High Point. The role involves providing comprehensive primary, palliative, and chronic disease management to home-bound patients, with responsibilities including physical examinations, diagnosis, treatment, care planning, and coordination with healthcare teams. Responsibilities

  • Deliver comprehensive primary care, palliative care, and chronic disease management to home-bound patients.
  • Conduct physical examinations, diagnose and treat illnesses, and manage chronic conditions.
  • Engage in advanced care planning discussions and coordinate care with other healthcare providers.
  • Build and maintain therapeutic relationships with patients and caregivers, providing emotional support and education.
  • Collaborate with interdisciplinary team members and communicate effectively with patients, caregivers, and healthcare professionals.
  • Travel to patients' homes within the service area to provide care.
  • Participate in on-call coverage for the Integrated Health Services population.
  • Focus on health promotion, disease prevention, and patient education.

Compensation

  • Salary-based compensation (W-2).

Requirements

  • Graduate of an accredited School of Nursing or ARC-PA accredited PA program.
  • Current Nurse Practitioner license in North Carolina.
  • Minimum of two years of experience in acute care, home health, long-term care, or outpatient/office setting.
  • CPR certification from the American Heart Association or American Red Cross BLS.
  • Excellent assessment and clinical skills, with strong interpersonal and communication abilities.
  • Valid state-issued driver's license and own transportation.
  • Must carry automobile liability insurance at agency-required limits.
  • Palliative care experience preferred.
