Join a dedicated Palliative Care team as a Nurse Practitioner or Physician Assistant, providing patient-centered care for individuals with complex, chronic, or life-limiting conditions. Collaborate with a multidisciplinary team to develop and implement individualized care plans, support patient self-management, and participate in quality improvement initiatives. This full-time, weekday position offers a supportive environment focused on delivering high-quality palliative care.

Responsibilities

• Develop and implement evidence-based care protocols for palliative patients.
• Assess, document, and analyze patient data to inform care decisions.
• Collaborate with physicians, nurses, social workers, chaplains, and other specialists to coordinate comprehensive care.
• Support patients and families in self-management, behavior change, and emotional support.
• Participate in quality improvement and performance evaluation activities.
• Manage a defined patient population according to established protocols and guidelines.
• Contribute to health screening programs and identify high-risk individuals.
• Ensure compliance with state licensure and certification requirements, including maintaining BLS certification.

Shift & Schedule

• Full-time, weekday schedule from 8:00 am to 4:30 pm.
• Monday through Friday, dayshift hours.
• No on-call responsibilities.

Requirements

• Master's Degree in Nursing with certification as a Family or Adult Nurse Practitioner.
• Minimum of 2 years' experience in Hospice or Palliative Care.
• Current licensure to practice nursing in Tennessee.
• BLS certification prior to start and ongoing maintenance.
• Graduate of a nationally accredited nursing school.
• Ability to develop individualized care plans based on patient data.
• Knowledge of growth and development principles across the lifespan.
• Strong collaboration and communication skills within a multidisciplinary team.
• Ability to interpret clinical data and assess patient needs according to age-specific requirements.
• Familiarity with evidence-based protocols and quality improvement processes.
