The Advanced Practice Clinician (NP or PA) will provide in-home assessments and clinical support to senior patients enrolled in the Care Anywhere program. The role focuses on chronic disease management, health monitoring, and patient education, with the goal of preventing hospitalizations and health complications. This full-time, remote position offers a Monday through Friday schedule with options for a 4/10 workweek, competitive compensation, and comprehensive benefits.

Responsibilities

  • Conduct in-home assessments, including history taking, physical exams, medication reviews, and screenings for cognitive, depression, and safety issues.
  • Obtain laboratory specimens in the home setting when appropriate.
  • Identify diagnoses to support active medical and care management of chronic conditions.
  • Communicate assessment findings to primary care providers to address care gaps.
  • Provide education to patients, families, and caregivers on health management and follow-up needs.
  • Utilize knowledge of community resources for patient referrals.
  • Discuss end-of-life care options with patients and families as needed.
  • Ensure compliance with HIPAA regulations and maintain the security of protected health information.

Compensation

  • Salary range: $130,332 to $195,498 annually, based on experience and qualifications.
  • Compensation model includes salary plus incentives, such as productivity bonuses.
  • Additional income opportunities: sign-on bonus, relocation support, CME allowance, and performance bonuses.
  • Tax status: W-2 employment.

Requirements

  • Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Active California Nurse Practitioner or Physician Assistant license, with a valid RN license and furnishing number (upon hire).
  • Board certification as a Nurse Practitioner or Physician Assistant.
  • Valid NPI number, DEA license, and BLS certification.
  • Valid California driver license and current automobile insurance.
  • Effective communication skills and professionalism in interactions.
  • Ability to problem-solve, organize, and manage time efficiently in a fast-paced environment.
  • Preferred: One year of clinical or home care experience, especially with geriatric patients and EMR systems.
  • Experience with end-of-life discussions is highly desirable.
