The Nurse Practitioner, specializing in Adult and/or Gerontology, provides high-quality primary care to residents in skilled nursing, assisted living, and independent living settings, as well as via telehealth. The role involves diagnosing, treating, and managing acute and chronic conditions, collaborating with interdisciplinary teams, and supporting a person-centered care model aimed at reducing hospitalizations and enhancing resident satisfaction. The position is based in Ohio, with responsibilities primarily during daytime hours on weekdays, with no on-call or weekend duties.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education to residents and families.
• Develop and communicate treatment plans to residents and other healthcare providers.
• Order, interpret, and adjust treatment based on lab and diagnostic results.
• Maintain compliant documentation within the electronic medical record (EMR).
• Collaborate with primary care providers, interdisciplinary teams, and telemedicine providers.
• Complete all regulatory visits and documentation as required.
• Participate in family conferences, advanced care planning, and address pharmacy communications.

Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, unrestricted state license and DEA license.
• Certification from AANP or ANCC.
• Minimum of 2 years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice.
• Ability to work in a flexible environment and schedule.
• US work authorization.
• Strong communication skills, detail orientation, and time management abilities.
