The Nurse Practitioner provides comprehensive hospice care under physician supervision, including patient assessment, diagnosis, care planning, medication management, and coordination with interdisciplinary teams. The role involves evaluating hospice eligibility, managing terminal illnesses, and supporting patients and families in community settings.

Responsibilities

• Provides medical care, manages terminal illnesses, and develops individualized care plans in compliance with state Nurse Practice Act and hospice regulations.
• Completes history and physical examinations, orders and interprets diagnostic tests, and documents findings in medical records.
• Prescribes medications and treatments, and makes referrals or consultations as needed.
• Performs face-to-face assessments to evaluate hospice eligibility and submits required documentation for recertification.
• Collaborates with the interdisciplinary team, attending physicians, and community healthcare providers to coordinate patient care.
• Communicates with patients, families, and caregivers to educate and counsel on medical, psychological, and end-of-life issues.
• Participates in hospice team meetings, maintains accurate clinical documentation, and ensures compliance with regulations.
• Maintains current knowledge of hospice regulations, quality improvement initiatives, and professional development.

Requirements

• Graduation from an accredited School of Nursing and Nurse Practitioner Program.
• Master's degree in Nursing with at least one year of Nurse Practitioner experience.
• Minimum one year of hospice or palliative care experience.
• Current nursing licensure in the state and CPR certification.
• Certification from an approved certifying body for advanced practice nursing (e.g., ANCC, AANP).
• Preferred certifications in specialty areas such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics.
• Excellent communication, problem-solving, and nursing skills as per competency standards.
• Ability to perform physical activities including standing, walking, lifting, and transferring patients.
• Visual and auditory acuity to perform clinical duties.
• Valid driver’s license, access to transportation, and ability to travel within the community.
