The Primary Care Provider (Nurse Practitioner or Physician Assistant) is responsible for delivering direct and indirect healthcare services to senior participants within the Senior TLC program. The role involves managing the health of chronically ill, geriatric patients in clinic, home, or community settings, under supervision of the Medical Director, and supporting team-based, patient-centered care aligned with the PACE model.

Responsibilities

• Perform health histories, physical exams, and patient education for senior participants.
• Order, interpret, and follow up on lab tests and diagnostic results.
• Initiate, evaluate, and modify medication and treatment plans within approved protocols.
• Assess, coordinate, and deliver direct participant care, including home and hospital visits.
• Request consultations from other healthcare services as needed.
• Collaborate with physicians, nurses, and other team members to support participant care and education.
• Participate in care planning, including setting SMART goals and documenting interventions.
• Attend interdisciplinary team meetings, supervisory sessions, and participate in on-call rotations.

Compensation

• Salary-based compensation with a specified salary scale.
• Sign-on bonus of $3,000.
• Compensation paid via W-2 tax status.

Requirements

• Master's Degree or higher as an Advanced Practice Provider from an accredited program.
• NC licensure as a Nurse Practitioner and Board Certification (ANCC).
• 3-5 years of medical experience, including at least 1 year as an APP.
• Experience working with frail or elderly populations.
• Ability to learn, teach, and research; problem-solving and analytical skills.
• Knowledge and skills necessary to treat geriatric patients.
• Must be medically cleared for all communicable diseases and have up-to-date immunizations.
• Ability to work within the scope of practice and adhere to organizational competencies.
• Strong teamwork and communication skills.
• Ability to perform procedures, emergency interventions, and participate in team-based care.
