The Nurse Practitioner at Miami Jewish Health's PACE program provides primary medical care, treatment, and counseling to elderly participants with complex medical needs. Under the supervision of the medical director, the role involves managing acute, episodic, and chronic illnesses, performing comprehensive assessments, and coordinating care with multidisciplinary teams in a geriatric and rehabilitation setting.

Responsibilities

• Performs comprehensive history and physical examinations on new referrals and quarterly on established participants.
• Manages patients with acute, episodic, and chronic illnesses.
• Assumes ongoing responsibility for each patient under care.
• Refers participants to medical director or specialists as indicated.
• Admits participants to hospital, provides primary care in collaboration with the medical director, and updates the multidisciplinary team regularly.
• Manages care of nursing home participants through visits, communication with staff, and hospital admissions when necessary.
• Participates in family conferences and interdisciplinary team meetings.
• Documents treatment in patient charts according to department guidelines.

Requirements

• Graduate from an accredited school of nursing.
• Current ARNP or PA license issued by the Florida State Board of Nursing.
• BLS certification; ACLS certification preferred.
• DEA license required.
• 2-3 years of experience preferred, with at least 1 year working with frail elderly populations.
• Knowledge of nursing care practices for geriatric, rehabilitation, and dementia patients preferred.
• Demonstrates a caring attitude toward residents, patients, families, and staff.
• Ability to assist with patient transfers involving lifting, pushing, and pulling of up to 150 pounds.
• Ability to work in environments with exposure to biohazards, chemicals, and infectious materials.
• Excellent communication skills and ability to maintain confidentiality.
