Advanced Practice Clinician

The Yakima Valley Farm Workers Clinic is seeking a full-time Advanced Practice Clinician to provide comprehensive, community-oriented primary care at the Lancaster Family Health Center in Salem, Oregon. The role involves managing a patient panel in a patient-centered medical home environment, serving underserved populations, and working within a team-based approach to deliver high-quality care. Responsibilities

• Manage a patient panel providing primary care services in a community health setting.
• Collaborate with a multidisciplinary team to deliver comprehensive patient care.
• Provide culturally competent care, including medical Spanish translation as needed.
• Participate in community health initiatives and patient education.
• Utilize the EPIC EMR system for documentation and care coordination.
• Work Monday through Friday, 7:30 am to 5:00 pm.
• Support underserved populations, including migrant and Latino communities.
• Engage in ongoing professional development and training, including a 4-week Medical Spanish Immersion Program.

Compensation

• $132,000 – $144,000 yearly base salary.
• Additional earnings from productivity and patient satisfaction measures.
• $15,000 signing bonus.
• $20,000 relocation allowance.
• Compensation is paid via W-2 with tax considerations.
• Includes malpractice insurance coverage (no tail coverage needed).

Requirements

• Master's or Doctoral Degree as a Nurse Practitioner or Physician Assistant.
• Two-year Physician Assistant training program or equivalent health science degree.
• Commitment to serving underserved and diverse patient populations.
• Bilingual in English and Spanish preferred.
• Experience or interest in community health and family medicine.
• Ability to work collaboratively within a team-based care model.
• Excellent communication and cultural competency skills.
• Willingness to participate in professional development activities.
• Current licensure and certification as required for practice in Oregon.
• Passion for providing compassionate, patient-centered care.
