Lifesong Hospice is seeking a licensed Nurse Practitioner to provide hospice and palliative care services in collaboration with the interdisciplinary team. The role involves developing care plans, prescribing medications, educating patients and families, and ensuring regulatory compliance across various patient care settings. Responsibilities

• Participate in developing and updating patient care plans that meet hospice standards and regulations.
• Provide clinical assessments, prescribe and adjust medications, and order diagnostic tests in collaboration with the IDG.
• Participate in rotating on-call availability for patient care needs.
• Complete accurate and timely electronic documentation to support reimbursement and compliance.
• Educate patients, families, and clinical staff on disease progression, symptom management, and medication stewardship.
• Maintain confidentiality and ensure compliance with federal and state regulations.
• Support ongoing professional development through continuing education and licensure requirements.
• Assist in maintaining high-quality, compassionate patient care and regulatory standards.

Requirements

• Current licensure as a Nurse Practitioner in Pennsylvania.
• Graduation from an accredited NP training program.
• Minimum of two years of clinical experience, preferably in geriatrics, home care, or hospice.
• Valid Pennsylvania driver’s license in good standing.
• Reliable automobile in good condition and proof of insurance.
• CPR certification.
• Proficiency with computers, EMR systems, and cellular technology.
• Self-directed with the ability to work independently with minimal supervision.
• Experience or certification in hospice and/or palliative care preferred.
