The Wellness Clinic Nurse Practitioner or Physician Assistant provides primary care services within Skyland Trail's Adult Intensive Services program. Responsibilities include conducting medical evaluations, developing treatment plans, prescribing medications, and coordinating care with interdisciplinary teams, patients, and families. The role emphasizes patient-centered care, safety, and adherence to clinical standards in a nonprofit mental health setting.

Responsibilities

  • Perform initial medical evaluations, preventive wellness visits, and symptom- or disease-focused assessments for patients.
  • Obtain medical histories, diagnose, and recommend evidence-based treatments with appropriate follow-up.
  • Consult with supervising physicians and communicate care plans to patients, families, and outpatient providers.
  • Coordinate outpatient referrals for urgent or specialty care as needed.
  • Prescribe medications legally authorized in Georgia, including counseling patients on drug regimens and side effects.
  • Document all clinical services accurately within 24 hours in the electronic health record.
  • Participate in treatment team meetings, quality improvement, safety, and infection control initiatives.
  • Develop service protocols and policies in collaboration with leadership.

Requirements

  • Experience with medication management, including prescribing and patient counseling.
  • Strong interpersonal and communication skills for effective team collaboration.
  • Proficiency in electronic health records, word processing, and email.
  • Ability to maintain safety, infection control, and quality standards.
  • Continued medical education to stay current with standards of care.
  • Knowledge of health education, accreditation, and certification requirements.
  • Clinical judgment to triage, treat, or refer patients appropriately.
