The Palliative Care Nurse Practitioner provides home-based primary and palliative care to patients with serious illnesses, utilizing telephonic and in-person consultations. The role involves leading advance care planning, collaborating with interdisciplinary teams, and educating patients to improve their health outcomes.

Responsibilities

• Provide high-quality home-based primary and palliative care through telephonic and in-person patient consultations, including traveling to patients' homes to manage advanced illness.
• Lead advance care planning discussions and prognosis with patients and families.
• Partner with physicians and an interdisciplinary team to develop and update comprehensive care plans considering medical, social, and environmental factors.
• Create detailed documentation focused on clinically relevant information to improve patient health and well-being.
• Educate patients on disease processes, medication management, and treatment compliance.
• Identify community resources and facilitate appropriate referrals as needed.

Requirements

• Current, unrestricted Nurse Practitioner license in the state of assignment.
• Possession of a Medicare Part B provider number or willingness to obtain one.
• Ability to obtain DEA licensure and prescriptive authority post-hire where applicable.
• Current State Driver's License, reliable transportation, and ongoing auto liability insurance.
• Nurse Practitioner national certification as ANP, FNP, or GNP.
• Clinical background in geriatrics, hospice, palliative medicine, or behavioral health preferred.
• Highly motivated, flexible, and adaptable to a dynamic work environment.
• Strong interpersonal skills to communicate effectively and build positive relationships.
