Address Healthcare of Michigan is seeking a part-time licensed Primary Care Nurse Practitioner or Physician Assistant to provide home-based primary care in the Grand Rapids, MI area. The role involves managing complex, acute, and chronic conditions for homebound patients, including conducting wellness exams, developing treatment plans, and collaborating with specialists and local providers. Responsibilities

• Perform age-appropriate history and physical examinations for complex, acute, critical, and chronically ill patients independently.
• Conduct annual wellness exams.
• Diagnose and develop treatment plans for homebound patients with acute, episodic, or chronic illnesses by analyzing medical histories, symptoms, physical findings, and diagnostic imaging.
• Maintain standards of care and adhere to professional practice guidelines.
• Collaborate with specialists to develop treatment plans for high-risk patients.
• Partner with local home health agencies, labs, imaging providers, and vendors.
• Communicate with patients and caregivers regarding self-management of diagnoses.
