Lower Cape Fear LifeCare is seeking a Nurse Practitioner or Physician Assistant to provide hospice and palliative care across southeastern North Carolina and northeastern South Carolina. The role involves patient visits in various settings, care coordination, and patient education, with a focus on delivering compassionate, quality care in a home and community-based environment.

Responsibilities

  • Conduct patient visits in personal residences, assisted living, and skilled nursing facilities, typically 4-5 visits per day, with flexibility to adjust based on patient needs.
  • Educate caregivers on disease management, medication administration, advance care planning, and end-of-life expectations.
  • Coordinate care with community medical providers and interdisciplinary team members, including physicians, nurses, and social workers.
  • Document patient encounters and care plans using provided equipment, including a laptop and company phone.
  • Drive between patient locations using personal vehicle, with mileage reimbursement.
  • Manage a patient caseload assigned by territory, with self-scheduling based on patient acuity and needs.
  • Participate in interdisciplinary team meetings twice a month and staff education sessions monthly.
  • Provide quality care regardless of socioeconomic or cultural background, adapting to changing patient needs.

Compensation

  • Salary paid via W-2 tax model.

Requirements

  • Master's Degree in Nursing or Physician Assistant from an accredited program.
  • Current certification from the American Academy of Nurse Practitioners (ANNP) or American Nurses Credentialing Center (ANCC).
  • Current licensure or ability to obtain licensure in North Carolina or South Carolina (NP).
  • Minimum of five years of general medical experience in inpatient, outpatient, or nursing home settings.
  • At least two years of experience as a nurse practitioner or physician assistant.
  • Experience seeing patients in home settings preferred.
  • Demonstrated leadership and teaching abilities.
  • Comfortable with patient education and care coordination.
  • Ability to drive and spend extended periods in a vehicle.
  • Excellent communication skills and cultural competence.
