Formed in 2022 through the merging of SCAN Group’s in-home program and The Residentialist Group, the practice partners with health plans and provider groups and operates in coordination with SCAN’s care delivery affiliates. Homebase Medical emphasizes in-home and virtual care delivery and works with advanced practice clinicians and clinical support staff to reduce hospital readmissions and support aging in place.
